HEALTH INFORMATION EXCHANGE (HIE)/PATIENT REGISTRY:

This feature aggregates patient data and maintains a patient registry in which other functions draw from. Once the data (some of it standardized much of it raw) is in the system the SHIFOX technology will normalize it so that it becomes usable and reportable. SHIFOX also has its own EMPI to properly identify patients when importing data.

POPULATION HEALTH MANAGEMENT:

Population Health is the analytics (health Informatics) for the system and executes all of the required reporting and calculating of quality metrics such as NCQA, CQM, PQRS, PCMH, DSRIP, AHRQ, ACO and CIGNA CAC. SHIFOX automatically calculates care gaps and patient risks based upon the patient chart and quality metric guidelines. The system also performs risk stratification to produce scores on patients for high costs and clinical risks. The system can manage various patient populations based on the guidelines set forth by different payer programs. Some of the examples of diseases include Diabetes Management, Respiratory Conditions, CAD, Preventive Services, Chronic Disease Care, General Care, Medication Management, etc.

CARE COORDINATION:

Customers are not only required to report on this data but they are also required to act upon it to improve medical outcomes. SHIFOX Care Coordination is a tool that will allow care coordinators to access the analytics and work with patients to help them improve their medical care and health. Some of the functions include: Automated tasking, automated clinical decision support, evidence-based protocols for chronic disease management, and ADT Notifications to alert doctors to when their patients are admitted to a hospital or facility and when they are discharged.

REFERRAL MANAGEMENT:

This feature is a robust offering that allows providers to manage their referrals with other physicians. This is a complex loop in which physicians are unaware and cannot track if patients are compliant with their orders. Referral management is integrated with the Care Coordination feature which is a significant part of the care management process.

CHRONIC CARE MANAGEMENT:

In January of 2015, CMS created a new payment mechanism in which Medicare pays $42/month/patient to providers who deliver 20+ minutes of non-face-to-face care coordination to Medicare beneficiaries with two or more chronic conditions. The SHIFOX solution provides practices with a robust set of tools necessary to document and manage the CCM process and earn new revenues.

HEALTH RISK ASSESSMENT/ANNUAL WELLNESS VISIT:

Medicare has a newer program that covers an Annual Wellness Visit (AWV) providing Personalized Prevention Plan Services for beneficiaries. Many managed care organizations are incentivized to complete an AWV or Health Risk Assessment. The AWV includes a Health Risk Assessment (HRA). The SHIFOX system provides an easy and intuitive feature to manage this function. The AWV is also required before signing up a CCM patient; the SHIFOX system is integrated to be able to conduct both functions and track together.

PATIENT ENGAGEMENT (Patient Portal):

SHIFOX offers customers an integrated patient portal for their care givers and coordinators to communicate directly with their patients. The portal includes Secure Messaging their Patient Chart and Care Plans, Patient Education, and Patient Surveys.

PRIVATE SOCIAL NETWORK:

The look and feel combined with social networking offers customers an overall communication tool that is integrated with the other features as well as securely communicate outside of the network. SHIFOX takes a unique approach in which it incorporates this feature with the normal workflow of the medical community which is more useful because providers are already on the system throughout the day.

Electronic Health Record (EHR):

SHIFOX has developed its own EHR to be able to improve the ability to interface, bi-directionally, with the physician practices. Also, many of the ACO type organizations do not have all of their members on an EHR system. SHIFOX has the ability to provide them with a cost effective option. SHIFOX is projecting to that this EHR will be MU certified by the end of 2016.

DIRECT MESSAGING:

Direct Messaging is the new standard to exchange protected health information between medical providers. SHIFOX is an EHNAC certified HISP and is a member of Direct Trust. This feature is integrated with all SHIFOX functions.
